Proactive Ergonomic Program

The County takes a proactive approach towards our employee's safety & health and is committed to reducing ergonomic injuries and maintaining a safe workplace.  In order to achieve our goals and assist in the transition back into the office, OSEC has expanded its services under the Proactive Ergonomic Program. In-person evaluations are now available to employees working in office or on County worksite.  Additionally, virtual ergonomic evaluations, done online, will continue to be offered to all employees including teleworkers. Evaluations are provided by vendors using OSEC funding.  For additional information or questions, please contact Tristan Robledo at [email protected] or 408-209-2635.

 Proactive Ergonomic Process (Process will vary)

  1. Employee must attain Supervisor approval, then complete the Proactive Ergonomic Evaluation Request Form (below).
  2. Email the form to [email protected]. The request may take 1 to 2 weeks to process. 
  3. When approved, an email will be sent to requestor and the supervisor of the employee having the evaluation.
  4. Instructions with accounting codes will be provided and must be used to create a Contract Release and Purchase Order (PO) number. 
  5. Once you have the PO number, contact the assigned vendor to schedule your appointment and send a copy to [email protected].  Please note, you cannot schedule without the PO number.
  6. The evaluation will include education, recommendations, and some possible modifications to the workstation.  Once completed, a report will be sent to the department, OSEC and designated coordinator (if applicable). Reports may contain equipment recommendations that are subject to the employee's department approval. 
  7. Once the report is received, requestor or department must notify their fiscal department to receive the PO so that payment can be made.  Any invoices must also be immediately sent to your fiscal department and to Tristan Robledo.
